Benchmarking medication error rates in palliative care services : not as simple as it seems

Medication errors are a leading cause of medical errors in palliative care services. Palliative care services frequently prescribe medications to control symptoms, including high risk medications such as opioids. There is an ever present inherent safety risk to all patients due to the complexity of the medication processes. Medication errors could potentially cause harm and death, particularly in patients with multiple comorbidities and limited physiological reserves. Medication errors could compromise effectiveness of symptom control, impact quality of life, incur additional resource utilisation and cause distress to patients and caregivers.
