L'auteur pense que les soins palliatifs ne peuvent pas être considérés comme des soins basiques du fait des compétences particulières qu'ils demandent pour une pratique de qualité. Les besoins en Afrique sont importants, mais ils ne peuvent pas être donnés uniquement par des spécialistes. Cette situation nécessite donc une adaptation tout en restant fidèle à la philosophie des soins palliatifs.
