International audience. Leptospirosis is a zoonosis caused by bacteria of the genus Leptospira. Few data are available to assess the clinical presentation and severity of human cases. French Guiana, Mayotte and Réunion are three French Overseas Departments with some of the highest incidences are among the highest in France. The aim of this study was to compare the severity of human cases of leptospirosis in these three departments . La leptospirose est une zoonose causée par les bactéries du genre Leptospira. Peu de données existent évaluant la présentation clinique et la sévérité des cas humains. La Guyane, Mayotte et La Réunion sont trois Départements d’Outre-Mer français dont les incidences sont parmi les plus élevées de France. L’objectif de ce travail était de comparer la sévérité des cas humains de leptospirose de ces trois départements
