En 2008, une unité de soins palliatifs nouvellement créée au Caire, en Egypte, a rencontré un cas complexe : un patient cancéreux qui vivait isolé sur une île uniquement accessible par ferry. L'équipe a due franchir de nombreux obstacles : règlementations restrictives, le manque d'éducation du personnel et de la famille, et la peur de mal utiliser les opioïdes
